What to expect on your first appointment
We will usually ask you to come in a little early so that you can fill out your new patient forms and health history. You can also download the forms below and bring them with you. We ask that you bring a list of all medications and your most current insurance information if you would like us to file for you.

Your first appointment will last approximately 1 hour. The hygienist will take x-rays and do a comprehensive exam to evaluate your tissue and tooth health. This will allow us to determine a treatment plan that will best fit your needs. Generally, we will also perform a routine cleaning and oral hygiene instructions. Dr. Mill will check existing restorations, diagnose necessary treatment and discuss any ongoing needs or concerns.
